Description: The RN implements professional nursing practice centered on the patient as a whole person. The focus is on creating a caring environment that promotes healing and wellness. Nursing practices are based on evidence and current research, as well as hospital policies and procedures, regulatory standards, and professional guidelines. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and across other disciplines. Responsible for coordinating, ensuring, and providing direct patient care activities on the unit. Primary responsibilities include provision of nursing rounds, assessments, and treatments. Facilitates admission, transfer, and discharge activities while providing patient and family-centered education. Engages in care coordination and individualized treatment planning as applicable. Essential functions include: - Performing and documenting comprehensive assessments according to nursing scope of practice. - Considering patients' culture, age, and developmental levels. - Identifying patient risk factors and presenting complaints. - Collecting both objective and subjective data. - Verifying initial admission assessments obtained by other healthcare team members. - Developing a problem list and prioritizing patient needs based on input from the inter-professional team as appropriate. - Designing actions and interventions relevant to patient culture and age. - Determining desired outcomes in coordination with patients and families in accordance with the medical plan of care. - Fully implementing patient care therapies and procedures while empowering patients within the established policies and professional guidelines. - Explaining procedures, treatments, and processes to patients and families. - Initiating actions to alleviate patient stress and discomfort. - Recognizing, responding to, and communicating changes in patient conditions appropriately. - Conducting and documenting ongoing assessments and responses to procedures and therapies using the nursing process according to hospital policy. - Delegating tasks appropriately and overseeing team members regarding patient care activities. - Monitoring the accuracy and completeness of delegated patient care. - Identifying and addressing educational needs of patients related to prioritized problems. - Facilitating the patient discharge process by executing discharge orders timely. - Providing input into patient/nurse assignments based on available staffing and patient acuity. - Actively participating in unit-based activities that support organizational values and priorities. - Promoting a positive group dynamic and healing environment through words and actions. - Assuring nursing excellence in accordance with accepted standards, evidence-based practice, and current research. - Participating in department or unit quality improvement activities. - Utilizing a holistic perspective in patient care.
