Job Description
We are seeking an experienced Home Health Registered Nurse (RN) for a travel assignment providing high-quality, patient-centered care in the home setting. The RN will manage a caseload of adult patients, perform comprehensive assessments, develop and implement care plans, and coordinate care to promote positive patient outcomes.
Responsibilities
- Perform Start of Care (SOC) assessments, recertifications, routine skilled nursing visits, and patient evaluations.
- Manage a caseload of approximately 30 adult patients.
- Complete an average of 6.5 patient points per day.
- Develop, implement, and update individualized care plans.
- Administer medications, perform wound care, and provide disease management and patient education.
- Accurately document all patient care using Homecare Homebase (HCHB).
- Collaborate with physicians, therapists, caregivers, and the interdisciplinary care team.
- Travel throughout the assigned coverage area to provide home health services.
