Home Health RN
Location: Plymouth County, Iowa
️ Schedule: Monday–Friday | Day Shift
⏰ Hours: 32–40 hours/week
On-Call: 1 in 4 nights, weekends & holidays
Lloyd Valley Healthcare is seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide high-quality care to patients in their homes throughout Plymouth County, Iowa.
Key Responsibilities- Provide skilled nursing care to patients in their homes.
- Perform OASIS assessments and develop individualized care plans.
- ❤️ Assess patient needs and provide appropriate clinical care.
- Coordinate care with patients, families, and the healthcare team.
- Participate in the department’s 1-in-4 on-call rotation, including nights, weekends, and holidays.
- Active Registered Nurse (RN) license.
- Minimum 2 years of experience performing OASIS assessments and care planning for home health patients.
- Previous Epic experience is a plus.
- ️ Strong clinical, communication, and organizational skills.
