Responsibilities:
- Assess home health patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Obtain data on physical, psychological, social, spiritual, and cultural factors influencing patient/family health and incorporate into the POC.
- Maintain accurate and up-to-date patient records reflecting visits, contacts, problems, plans, actions, and goals.
- Provide care utilizing infection control measures compliant with OSHA standards.
- Educate patients and families on care, medications, diet, home safety, and self-care practices appropriate to their age, condition, culture, and learning readiness.
- Supervise and provide clinical direction to HHA/STNA/LVN staff to ensure quality and continuity of service.
- Reassess pain levels, utilize appropriate management techniques, and educate on pain management.
- Arrange referrals to appropriate agencies as needed.
- Monitor cases to ensure compliance with third-party payers’ requirements.
- Perform on-call duties, providing emergency services to patients and families.
- Communicate effectively as a liaison among patients, families, team members, physicians, and other healthcare professionals.
- Attend interdisciplinary team meetings as appropriate.
- Perform other related duties as assigned.
