JOB DESCRIPTION
About Facility:
o Home Health – 901 N. 2nd St., Albemarle, NC 28001
o Radius rule: Traveler's permanent address must be 50 miles outside of home unit
Department Information:
o Teammates are required to complete 6 visits a day (routine visits, discharge, ROC, recert, reassessment)
o Minimum Years of Experience required? 1 year Home Health experience
o Will you accept a first-time traveler? Yes, if seasoned to home health
Patient Types/Common Diagnoses:
o Chronic diseases, wound care, IV, pleurx, wound vac
o There is not a max mileage to drive. The average is 60-80 miles a day. Mileage is reimbursed weekly.
o Teammates will travel throughout the below areas:
Albemarle, Badin, Harrisburg, Mt Gilead, Norwood, Troy and cities in between
Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or Insurance:
o Driver's License and proof of Insurance
o BLS
Skills required:
o Homecare Homebase experience is required (no exceptions), IV, pleurx, wound vac, wound care, assessment, disease teaching, OASIS, case management (ability to oversee/episode management up to 25-30 patients at once)
Support within the Department:
o Departmental leadership
o Schedular
o Clinical Manager
o Clinical Supervisor
o Peers/Clinical Team
Technology/Equipment:
o EPIC Rover – view only
o Focura - will upload discharge instructions and other important docs such as wound pictures
o Samsung note 9 phones
o EMR: HCHB (Homecare Homebase) – experience required
Orientation (facility and unit):
o NEO
o 5 days orientation
Scheduling:
o Weekend rotation: none, unless willing and interested in working weekends
o Call is not required, but may be available if interested
o Holiday Expectations: Travelers will work 75% of holidays
o RTO: Manager approval
o Schedule cycle: every 13 weeks
