Travel Nurse RN - Home Health - $1,988 per week in Maplecrest, NY

Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Home Health Duration: Temporary Shift: Part Time Hours per Shift: Approximately 2 Face-to-Face visits per week Experience: Two years of experience in a home setting within the last fifteen years License: Current RN Licensure in Client Certifications: BLS certification required before the start date Must-Have: - Recent experience conducting comprehensive assessments of patients - Ability to work with technology in the workplace - Knowledge of the theory, principles, and practices of general professional nursing - Excellent organizational, interpersonal, written, and verbal communication skills - Ability to communicate and work effectively with multi-generational consumers - Sensitivity to the concerns of others - Ability to perform comfortably in a fast-paced, deadline-oriented work environment - Strong ability to work independently Description: Perform initial in-home pre-evaluation of patients that includes evaluation of health status, strengths, care needs, and preferences. Guide the development of individualized long-term care service plans. Review consumer medical documentation and health referral forms, as relevant to the case. Enter evaluation data into electronic evaluation form and transmit as required. Document any concerns or conflicting information that surfaces during the evaluation process. Emphasize continuity of care to reduce fragmentation, duplication, and gaps in the treatment plan. Discuss health care options, supports needed, service vendor options, and waiver options with the consumer. Initiate communication between eligible consumers for enrollment into a managed care health plan. Enhance communication and collaborative relationships with interdisciplinary care team members. Collect quality review data and required documentation for outcome measurements and record case notes. Identify opportunities for health promotion and illness prevention. Maintain knowledge of community resources, payor requirements, and network services for the target population. Meet all standards established for this position as outlined in the annual performance criteria. Perform other duties as assigned by the Regional Nurse Manager or project management.
