Must-Have:
- Experience conducting comprehensive assessments of patients
- Experience working in a certified home health agency, licensed home care service agency, or managed long-term care
- Experience caring for a minimum of two patients per week
Description:
The Registered Nurse will be responsible for performing initial in-home pre-evaluations of patients.
This includes evaluating health status, strengths, care needs, and preferences.
The nurse will guide the development of individualized long-term care service plans.
Responsibilities include reviewing consumer medical documentation and health referral forms as relevant to each case.
The nurse will enter evaluation data into electronic forms and transmit as required.
Documentation of any concerns or conflicting information that arises during the evaluation process is essential.
The focus will be on emphasizing continuity of care to reduce fragmentation, duplication, and treatment plan gaps.
The nurse will discuss healthcare options, supports needed, service vendor options, and waiver options with consumers.
Communication will be initiated between the call center and eligible individuals for enrollment into a managed care health plan.
Enhancing communication and relationships with interdisciplinary care team members will improve care coordination.
Collecting quality review data and necessary documentation to support outcome measurements is part of the role.
The nurse will identify opportunities for health promotion and illness prevention.
Maintaining a comprehensive working knowledge of community resources and payer requirements is necessary.
The employee must meet all performance standards established for this position.
Additional duties may be assigned by the Regional Nurse Manager or other project management.
Candidates must possess organizational, interpersonal, written, and verbal communication skills.
A strong ability to work independently and perform in a fast-paced environment is required.
The ability to communicate effectively with multi-generational consumers is essential.
Demonstrating sensitivity to the concerns of others is vital.
