Description:
Urgent need for a part-time position (approximately 2 face-to-face visits per week).
Candidates must be prepared to conduct hybrid telehealth and in-field assessments.
Candidates must maintain a comprehensive understanding of the theory, principles, and practices of professional nursing.
Strong organizational, interpersonal, written, and verbal communication skills are required.
Ability to communicate and work effectively with diverse consumers is essential.
Candidates must demonstrate sensitivity to the concerns of others.
Ability to perform comfortably in a fast-paced, deadline-oriented work environment is necessary.
Candidates should be adept at successfully executing multiple complex tasks simultaneously.
Independent work ability is crucial.
Responsibilities include performing initial in-home evaluations of patients.
Evaluation includes assessing health status, strengths, care needs, and preferences.
Development of individualized long-term care service plans is guided by these evaluations.
Candidates will review consumer medical documentation and health referral forms as relevant to cases.
Evaluation data must be entered into electronic documentation systems and transmitted as required.
Any concerns or conflicting information that surfaces during the evaluation process must be documented.
Emphasis on continuity of care is needed to reduce fragmentation, duplication, and gaps in treatment plans.
Discussions with consumers regarding healthcare options, needed supports, and service vendor options will be required.
Initiating communication between relevant parties for enrollment into managed care health plans is essential.
Enhancement of communication and collaborative relationships with interdisciplinary care teams is expected.
Collection of quality review data and documentation to support outcome measurements will be part of the role.
Candidates must identify opportunities for health promotion and illness prevention.
A comprehensive working knowledge of community resources and payor requirements for the target population is essential.
Meeting all performance standards established for this position is necessary.
Other duties may be assigned by management as needed.
