- Experience providing primary care to homebound individuals and care management for persons receiving home and community-based services.
Description:
The role requires the ability to review health status questions and make evaluations based on UAS criteria for enrollment.
Candidates must be comfortable working with technology in the workplace.
Knowledge of the principles and practices of professional nursing is essential.
Understanding state and agency regulations governing nursing practices is necessary.
Strong organizational and communication skills are required, including the ability to communicate effectively with a diverse population.
Candidates should possess sensitivity to the concerns of others.
The ability to work independently in a fast-paced, deadline-oriented environment is crucial.
The job includes performing initial in-home evaluations of patients.
Responsibilities involve reviewing consumer medical documentation relevant to each case.
Data entry into electronic forms and documentation of any concerns during the evaluation process are required.
Emphasizing continuity of care to reduce gaps in treatment plans is a key responsibility.
Candidates will discuss healthcare options and supports needed with consumers.
Initiating communication for enrollment into managed care plans is part of the role.
Enhancing relationships with interdisciplinary care team members is important for improving care coordination.
Collecting quality review data and managing required documentation is necessary.
Identifying opportunities for health promotion and illness prevention is expected.
Maintaining knowledge of community resources and payor requirements is essential.
Meeting established performance standards for this position is required.
