Description:
Current RN Licensure in Client required.
Two (2) years of experience in a home health setting within the last ten (10) years is mandatory.
Candidates must have recent experience conducting comprehensive assessments of patients.
Qualified candidates should have worked for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care.
Experience should include conducting assessments similar to OASIS.
Providing primary care to homebound persons is a key requirement.
Additionally, candidates must have experience in caring for a minimum of two patients per week.
Preferred candidates will have experience conducting assessments in UAS-NY, especially in the last 365 days.
Experience caring for a geriatric population and managing chronic diseases in the home setting is preferred.
Combining home care experience as a paraprofessional or LPN with RN experience is a strong advantage.
Job responsibilities include performing initial in-home pre-evaluation of patients.
Candidates will evaluate health status, strengths, care needs, and preferences to guide individualized care plans.
Reviewing consumer medical documentation and health referral forms relevant to each case is necessary.
Candidates must enter evaluation data into electronic forms and transmit as required.
Documenting concerns and conflicting information during evaluations is essential.
Emphasizing continuity of care to reduce fragmentation, duplication, and gaps in treatment is a responsibility.
Candidates will discuss health care options, necessary supports, and service vendor options with consumers.
Initiating communication between eligible consumers for enrollment into managed care is required.
Enhancing collaborative relationships with interdisciplinary care team members is critical for service delivery.
Candidates will collect quality review data and document case notes as part of their role.
Identifying opportunities for health promotion and illness prevention is expected.
Maintaining knowledge of community resources and payor requirements is necessary.
Meeting all performance standards for this position is imperative.
Other duties may be assigned by the Regional Nurse Manager or project management as needed.
