Job Summary:
Home Healthcare Registered Nurse position in Lorain, Ohio. This role involves providing in-home care to geriatric patients, with a schedule of Monday through Friday, 8:00 AM to 5:00 PM. The position requires flexibility with territory and offers the opportunity to work within a team dedicated to delivering quality home health services.
Job Details:
- Location: Lorain, Ohio
- Schedule: Monday - Friday; 8 hours per day, minimum of 40 hours per week
- Required Qualifications:
- One (1) year of relevant experience in home health care
- Active license in Ohio
- Proficiency in Homecare Homebase (HCHB) required
- Experience with OASIS assessments required for staff completing Start of Care (SOC)
- BLS certification required
- Recent experience with geriatric patient populations preferred
- Preferred Qualifications:
- Community hospital experience
- Home health or hospice experience preferred
- Additional Information:
- On-call rotation may be required, to be discussed during interview
- Timely documentation and communication are mandatory
- Mileage will be reimbursed at IRS standard rate
- All travelers must have an active driver's license and personal transportation
- RTO (Requested Time Off) must be submitted at the time of application
- No first-time travelers accepted
- All candidates must be flexible with territory within up to a 60-mile radius of the designated hub/office
- Orientation provided on Mondays, with mandatory modules to be completed during orientation
