Description:
Required licensure: Current RN licensure in Client.

Experience may include working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care.
Conducting assessments that are similar to the UAS such as OASIS is preferred.
Providing primary care to homebound persons is essential.
Care management for persons receiving home and community-based services is required.
Home care experience as a paraprofessional or Licensed Practical Nurse is considered valuable.
Preferred qualifications include experience conducting assessments in UAS-NY, especially in the last 365 days.
Experience managing chronic diseases in a geriatric population is preferred.
Candidates must be prepared to conduct hybrid tele and in-field assessments.
Ability to review health status questions and make evaluations based on UAS criteria is necessary.
Strong knowledge of nursing principles, state laws, and regulations governing nursing practices is essential.
Excellent organizational, interpersonal, written, and verbal communication skills are required.
Candidates must demonstrate sensitivity to the concerns of others.
Ability to work effectively in a fast-paced, deadline-oriented environment is necessary.
The capacity to manage multiple complex tasks simultaneously is essential.
