License: Current RN Licensure in Client Certifications: BLS (required before the start date) Must-Have: Recent experience conducting comprehensive assessments of patients Description: Urgent need for a Registered Nurse in the home health specialty. Candidates must have a current RN licensure in Client. Individuals should possess two years of experience in a home health setting within the last fifteen years. Recent experience conducting comprehensive assessments of patients is required. Ability to review health status questions and make evaluations and determinations based on criteria for enrollment. Proficiency in working with technology in the workplace is necessary. Knowledge of the theory, principles, and practices of general professional nursing is essential. Understanding state and agency laws and regulations governing nursing practices is required. Candidates must demonstrate excellent organizational, interpersonal, written, and verbal communication skills. The ability to communicate and work effectively with multi-generational consumers is important. Sensitivity to the concerns of others is crucial. Candidates should perform comfortably in a fast-paced, deadline-oriented work environment. The ability to manage multiple complex tasks simultaneously is needed. A strong ability to work independently is required. Responsibilities include performing initial in-home evaluations of patients. Evaluate health status, strengths, care needs, and preferences. Guide the development of individualized long-term care service plans. Review consumer medical documentation and health referral forms as relevant. Enter evaluation data into electronic forms and transmit as required. Document any concerns or conflicting information that arises during evaluations. Emphasize continuity of care to reduce fragmentation, duplication, and gaps in treatment plans. Discuss healthcare options, necessary supports, and service vendor options with consumers. Initiate communication for consumer enrollment into a managed care health plan. Enhance communication and collaborative relationships with interdisciplinary care team members. Collect quality review data and necessary documentation to support outcome measurements. Identify opportunities for health promotion and illness prevention. Maintain knowledge of community resources, payor requirements, and network services for targeted populations. Meet established standards for this position. Perform other duties as assigned by management. Candidates must be prepared to conduct in-field assessments.
