Job Title: RN - Home Health Profession: Registered Nurse (RN) Specialty: Home Health Duration: 260 days Shift: Day Hours per Shift: 7.5 Hours Experience: Minimum of Two (2) years of experience in a home health setting within the last fifteen (15) years. License: Current RN Licensure in Client Certifications: BLS required before start date Must-Have: Recent experience conducting comprehensive assessments of patients Description: We have an urgent need for a part-time RN in the home health specialty. This position will involve approximately two face-to-face visits per week. Candidates must have experience conducting comprehensive patient assessments. Nurses will conduct initial in-home pre-evaluations of patients, which will include health status evaluation, strengths, care needs, and preferences. A critical function of this role is the development of individualized long-term care service plans. Nurses are expected to review consumer medical documentation and health referral forms relevant to each case. Evaluation data must be entered into an electronic evaluation form and transmitted as required. Any concerns or conflicting information identified during evaluations must be documented. Emphasis on continuity of care is essential to reduce fragmentation and duplication within treatment plans. Candidates will discuss health care options, needed supports, service vendor options, and waiver options with consumers. It is important to initiate communication between the call center and eligible consumers for enrollment into managed care health plans. Enhancing communication and collaborative relationships with interdisciplinary care team members is necessary to improve care coordination and facilitate service delivery. Quality review data and required documentation should be collected to support outcome measurements and record case notes. Identifying opportunities for health promotion and illness prevention is a key responsibility. Maintaining comprehensive knowledge of community resources, payor requirements, and network services for the target population is essential. Candidates must meet all standards established for the position as outlined in the performance criteria. Additional duties may be assigned by the Regional Nurse Manager or project management as needed.
