Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: Temporary Shift: Day Hours per Shift: 7.5 Experience: Two (2) years of experience in a home setting within the last fifteen (15) years License: Current RN Licensure in Client Certifications: Basic Life Support (BLS) required before the start date Must-Have: - Recent experience conducting comprehensive assessments of patients - Familiarity with technology in the workplace - Knowledge of general professional nursing principles, theories, and practices - Excellent organizational, interpersonal, written, and verbal communication skills - Ability to communicate effectively with a diverse population - Strong capability to work independently and manage multiple tasks in a fast-paced environment Description: We have an urgent need for part-time Registered Nurses to conduct face-to-face visits. Nurses are expected to perform initial in-home evaluations of patients. This includes assessing health status, strengths, care needs, and preferences. The role guides the development of individualized long-term care service plans. Nurses will review medical documentation and health referral forms relevant to cases. Evaluation data must be entered into electronic forms and transmitted as required. Documentation of any concerns, conflicting information, or issues during assessments is essential. Continuity of care is emphasized to reduce fragmentation, duplication, and gaps in treatment plans. Nurses will discuss healthcare options with consumers, including supports and service vendor options. Initiating communication between eligible consumers for enrollment into managed care is required. Enhancing collaborative relationships with interdisciplinary team members to improve care coordination is vital. Quality review data must be collected along with required documentation to support outcome measurements. Opportunities for health promotion and illness prevention should be identified. A comprehensive knowledge of community resources, payer requirements, and network services for the target population is necessary. All standards for the position as outlined in performance criteria must be met. Additional duties may be assigned by the Regional Nurse Manager or project management. All candidates must be prepared to conduct in-field assessments.
