Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Home Health Duration: 260 days Shift: Day Hours per Shift: 7.5 Experience: Two years of experience in a home health setting within the last fifteen years License: Current RN Licensure in Client Certifications: BLS certification is required before start date Must-Have: Recent experience conducting comprehensive assessments of patients Description: Urgent need for part-time Registered Nurse. The position requires approximately two face-to-face visits per week. Candidates must be prepared to conduct hybrid telehealth and in-field assessments. Nurses must have the ability to review health status questions and make evaluations based on criteria for enrollment. Candidates should be proficient in working with technology in the workplace. Knowledge of the theory, principles, and practices of general professional nursing is essential. Familiarity with state and agency laws and regulations governing nursing practices is required. Candidates must possess excellent organizational, interpersonal, written, and verbal communication skills. The ability to communicate and work effectively with diverse consumers is important. Sensitivity to the concerns of others is necessary. Candidates should be comfortable performing in a fast-paced, deadline-oriented environment. The ability to successfully execute complex tasks simultaneously is required. Nurses must have a strong ability to work independently. Responsibilities include performing initial in-home evaluations of patients. Evaluations will include assessments of health status, strengths, care needs, and preferences. The development of individualized long-term care service plans will be guided by these evaluations. Reviewing consumer medical documentation and health referral forms relevant to cases is required. Evaluation data must be entered into an electronic evaluation form. Candidates will document any concerns or conflicting information that arises during the evaluation process. Emphasis on continuity of care is essential to reduce fragmentation and gaps in treatment plans. Discussing healthcare options and supports needed with consumers is expected. Initiating communication for enrollment into health plans is a critical task. Enhancing communication with interdisciplinary care team members to improve care coordination is vital. Collecting quality review data and required documentation for outcome measurements is part of the job. Identifying opportunities for health promotion and illness prevention will be part of the responsibilities. Maintaining a comprehensive knowledge of community resources and services for the target population is required. Meeting all standards established for this position as outlined in performance criteria is necessary. Other duties may be assigned by management as needed.
