Job Title: Home Health RN Profession: Registered Nurse Specialty: Home Health Duration: 260 days Shift: 5x5 days Hours per Shift: Guaranteed hours not specified Experience: Two (2) years of experience in a home setting within the last ten (10) years License: Current RN Licensure in Client Certifications: BLS required before start date Must-Have: - Two (2) years of experience in a home setting within the last ten (10) years. - Experience conducting comprehensive assessments of patients. Description: Required licensure is a current RN licensure. Required experience includes two years in a home setting, which may include working for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Candidates must have recent experience conducting assessments similar to UAS, such as OASIS. Primary care experience for homebound persons and care management for those receiving home and community-based services is necessary. Home care experience as a paraprofessional or Licensed Practical Nurse, caring for a minimum of two patients per week, is also required. Preferred qualifications include experience in conducting assessments in UAS within the last 365 days and experience managing chronic diseases in geriatric populations. Combination of home care experience as a paraprofessional or LPN, along with RN experience, is preferred. Other examples of experience may include private duty nursing and case management limited to utilization reviews. Candidates must be prepared to conduct both telehealth and in-field assessments. Candidates will sign a COI form before starting. Ability to review health status questions and make evaluations based on UAS criteria for enrollment is essential. Candidates should have knowledge of professional nursing principles and practices. Excellent organizational and communication skills are necessary to work effectively with multi-generational consumers. Candidates should demonstrate sensitivity to the concerns of others and perform well in fast-paced environments. Responsibilities include performing initial in-home pre-evaluations of patients, reviewing medical documentation, and documenting any concerns that arise during evaluations. Continuity of care and effective communication with interdisciplinary team members are vital for improving care coordination. Collecting quality review data and maintaining a comprehensive knowledge of community resources is expected. Candidates are required to meet all performance standards and may be assigned other duties as necessary.
