Recent experience conducting comprehensive assessments of patients. Experience conducting assessments that are similar to UAS, such as OASIS. Experience providing primary care to homebound individuals. Experience in care management for individuals receiving home and community-based services. Experience caring for a minimum of two patients per week. Description: Required licensure includes a current RN licensure in New York State. Ideal candidates will have two years of experience in a home setting within the last ten years. Experience can include working for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Candidates should be experienced in conducting assessments that inform service plans. Preferred candidates will have experience with UAS-NY assessments, particularly within the last year. Experience managing chronic diseases in a geriatric population in a home setting is desirable. A mix of home care experience as a paraprofessional or Licensed Practical Nurse while caring for a minimum of two patients weekly is preferred. Candidates must demonstrate ability to review health status questions and make evaluations based on UAS criteria for enrollment. Candidates should be proficient in using technology in the workplace and knowledgeable about nursing theories, principles, and practices. Excellent organizational, interpersonal, written, and verbal communication skills are essential. Candidates should be capable of working effectively with diverse consumers. Sensitivity to others' concerns and the ability to thrive in a fast-paced environment are important. Responsibilities include performing initial in-home evaluations of patients and evaluating their health status, strengths, care needs, and preferences. Candidates will review consumer medical documentation relevant to cases. Data must be entered into electronic evaluation forms and transmitted as required. Documenting any issues that arise during evaluations is necessary. Emphasis will be placed on continuity of care to minimize fragmentation and gaps in treatment. Candidates will discuss health care options and supports needed with consumers. Candidates will facilitate communication between the call center and eligible consumers for enrollment into managed care. Enhancing communication and collaborative relationships with interdisciplinary care team members is key to improving care coordination. Collecting quality review data and documenting case notes in CRM is required. Opportunities for health promotion and illness prevention should be identified. Maintaining knowledge of community resources, payor requirements, and services for the target population is necessary. Adhering to the established performance standards for this position is expected. Additional duties may be assigned by the Regional Nurse Manager or project management.
