Job Title: RN Profession: Nursing Specialty: Home Health Duration: 260 days Shift: 5x5 Days Hours per Shift: 8 Experience: Minimum of two years in a home health setting within the last ten years License: Current RN Licensure in New York State Certifications: BLS required before the start date Must-Have: - Two years of experience in a home health setting within the last ten years - Experience conducting comprehensive patient assessments - Ability to communicate and work effectively with multi-generational consumers - Knowledge of the theory, principles, and practices of general professional nursing Description: Required licensure is a current RN licensure in New York State. Candidates must have a minimum of two years of experience in a home setting within the last ten years. This includes working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Recent experience in conducting comprehensive assessments of patients is essential. Nurses are expected to perform initial in-home pre-evaluations of patients. This includes evaluating health status, strengths, care needs, and preferences. The development of individualized long-term care service plans is guided by these evaluations. Reviewing consumer medical documentation and health referral forms is part of the responsibilities. Data from evaluations must be entered into an electronic form and transmitted as required. Documenting any concerns, conflicting information, or issues that arise during the evaluation process is necessary. Nurses will discuss health care options, supports needed, and service vendor options with consumers. Communication between the Call Center and eligible consumers for enrollment into managed care health plans must be initiated. Enhancing communication and collaborative relationships with interdisciplinary care team members is vital. Collecting quality review data and required documentation to support outcome measurements is expected. Opportunities for health promotion and illness prevention must be identified. Maintaining a comprehensive knowledge of community resources and payor requirements is essential. Meeting all standards established for this position is required. Other duties may be assigned by the Regional Nurse Manager or project management.
