License: Current RN Licensure in New York State Certifications: BLS (required before the start date) Must-Have: - Two (2) years of experience in a home health setting. - Experience conducting comprehensive assessments of patients. - Ability to perform in-home evaluations and develop individualized long-term care service plans. Description: Required licensure is a current RN licensure. Two (2) years of experience in a home setting within the last ten (10) years is required. Experience may include working for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. Candidates must have recent experience conducting assessments similar to UAS, such as OASIS. Providing primary care to homebound individuals and managing care for those receiving home and community-based services is essential. Experience caring for a minimum of two patients per week is necessary. Preferred qualifications include experience conducting assessments in UAS-NY, especially within the last 365 days. Additional experience caring for a geriatric population and managing chronic diseases in a home setting is valued. Home care experience as a paraprofessional or LPN is beneficial. Candidates must be prepared to conduct hybrid telephonic and in-field assessments. Assessments must be performed in-person; candidates cannot conduct UAS assessments elsewhere. All candidates must demonstrate excellent organizational, interpersonal, written, and verbal communication skills. The ability to work independently in a fast-paced environment is essential. Candidates should have the capability to perform multiple complex tasks simultaneously. Responsibilities include performing initial in-home evaluations of patients, reviewing medical documentation, and entering evaluation data into electronic forms. Candidates will document concerns and emphasize continuity of care. Discussing healthcare options and initiating communication between care teams and eligible consumers are part of the role. Enhancing communication with interdisciplinary care team members to improve coordination and facilitate service delivery is vital. Collecting quality review data and maintaining knowledge of community resources and payor requirements will be required. Candidates should meet all established standards for performance and complete other duties as assigned.
