TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Lewis, New York, 12950!

Job Title: Registered Nurse Profession: RN Specialty: Home Health Duration: 260 Days Shift: Day Hours per Shift: 7.5 Experience: Two (2) years of experience in a home health setting within the last fifteen (15) years. License: Current RN Licensure in Client Certifications: Current BLS certification is required before the start date. Must-Have: - Recent experience conducting comprehensive assessments of patients - Ability to work with technology in the workplace - Knowledge of the theory, principles, and practices of general professional nursing - Excellent organizational, interpersonal, written, and verbal communication skills - Ability to communicate and work effectively with multi-generational consumers - Strong ability to work independently Description: Urgent need for a part-time RN for approximately 2 face-to-face visits per week. The position requires the ability to perform initial in-home pre-evaluation of patients. This includes evaluating health status, strengths, care needs, and preferences to guide the development of individualized long-term care service plans. Candidates will review consumer medical documentation and health referral forms, as relevant to the case. Evaluation data must be entered into an electronic form and transmitted as required. It is essential to document any concerns, conflicting information, or issues that surface during the evaluation process. Ensuring continuity of care is crucial, thus reducing or eliminating fragmentation, duplication, and gaps in the treatment plan. Candidates will discuss healthcare options, supports needed, service vendor options, and waiver options with consumers. Initiating communication between the call center and eligible consumers for enrollment into a managed care health plan is required. Enhancing communication and fostering collaborative relationships with interdisciplinary care team members will improve care coordination and facilitate service delivery. Collecting quality review data and required documentation to support outcome measurements is necessary. It is important to maintain a comprehensive working knowledge of community resources, payer requirements, and network services for the target population. Candidates must meet all standards established for this position as outlined in the annual performance criteria. Other duties may be assigned by the Regional Nurse Manager or project management. All candidates must be aware that they will be conducting in-field assessments.
