Must-Have:
- Must be proficient in HomeCare HomeBase and OASIS and used it within the last 6 months.
- Must have reliable transportation, a current driver's license, and agency-required liability insurance.
- Must be open to traveling up to 75 miles from the assigned care center.
- Must understand that all documentation must be completed within 24 hours of the appointment with the patient.
- Must accept every patient on the schedule regardless of distance from previous appointments.
- Must be comfortable with patient home visits and attending periodic meetings.
Description:
Provides coordinated and directed home nursing care according to physician orders.
The care is based on established agency policies and procedures.
The role involves the competent application of the nursing process.
Responsible for supervising and evaluating home health aides and licensed practical nurses or licensed vocational nurses.
Average patient visits per day range from 5 to 10.
Documentation and travel activities hold different point values.
No guaranteed hours are provided, and weekly productivity points must be met.
