Must-Have:
HCHB (Home Care Home Base) experience is a hard requirement.
RNs are expected to conduct "start of care," recertifications, and daily visits.
RNs will case manage approximately 30 adult patients per day with a point system of 6.5 points.
A clinical manager or educator is available for emergencies 24/7 or from 8:00 AM to 5:00 PM on weekdays.
There is a holiday rotation requirement, and weekend shifts will be determined based on the number of nurses in the rotation.
Description:
The Home Health Registered Nurse will provide care to adult patients in a home health setting.
They will conduct assessments, treatments, and patient education as needed.
This position does not require managing home vents.
Scrubs are required, but there is no specified color.
Orientation will include two weeks with a preceptor.
No call requirements are noted for this position.
