Job Title: Hospice RN, Home Health RN, Wound Care

License: Must have a driver’s license in good standing
Certifications: BLS required
Must-Have:
- OASIS (Outcome and Assessment Information Set) experience
- Home health and hospice experience
- Experience traveling from patient to patient for home visits
- Ability to manage and adjust to changes in schedule
- Familiarity with EMR system used (HealthWyse)
Description:
This is a full-time position with a shift from Monday to Friday, 8 am to 5 pm.
Some weekend coverage is required.
Call may be scheduled, but only on an emergency basis.
The role involves providing outpatient care.
You will need to travel to patient locations for home visits.
A background check will be conducted.
