Travel Nurse RN - Home Health - $2,262 per week in Plymouth, MA

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficient in HomeCare HomeBase and Oasis Description: Provides, coordinates, and directs the provision of home nursing care according to physician's orders. Nursing care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits are required, along with periodic office meetings. Must have reliable transportation and current driver's license. Agency-required liability insurance is necessary. Average number of patients per day is between 5 to 8. Contractors will be assigned between 5 to 10 patients per day, equating to roughly 6 to 8 productivity points. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Must be comfortable traveling up to 75 miles for home visits. Local candidates are accepted at the same rate. Experience in Home Health is required, and first-time travelers are not accepted. No on-call shifts required every week, but there may be a rotation every fourth week. Documentation must be completed within 24 hours of the patient appointment. Expectations include accepting all patients on the schedule regardless of distance. No guaranteed hours are offered with this contract. Weekly productivity points of 30 are expected, equating to 40 hours. Immediate float opportunities may be available if patient numbers are low.
