Job Summary
We are seeking a dedicated and experienced Home Health Registered Nurse to provide comprehensive, in-home patient care within Plymouth County, Iowa. This role is based out of Le Mars, Iowa, and focuses on delivering high-quality nursing services to patients in their residential settings.
The successful candidate will be responsible for managing a caseload of patients, conducting thorough assessments, developing individualized care plans, and ensuring continuity of care. This position requires a compassionate professional with strong clinical skills and the ability to work independently while collaborating with a multidisciplinary team.
Key Responsibilities
- Provide skilled nursing care to patients in their homes, adhering to established care plans and physician orders.
- Conduct comprehensive OASIS assessments to determine patient needs and eligibility for home health services.
- Develop, implement, and evaluate individualized patient care plans, adjusting as necessary to meet changing patient conditions.
- Monitor patient health status, administer medications, perform treatments, and educate patients and families on disease management and prevention.
- Maintain accurate, timely, and complete electronic health records in compliance with regulatory standards.
- Collaborate effectively with physicians, therapists, social workers, and other healthcare professionals to ensure coordinated patient care.
- Participate in the departmental call rotation, providing support for nights, weekends, and holidays.
Required Qualifications
- Active Registered Nurse (RN) license.
- Minimum of two (2) years of recent experience performing OASIS assessments and care planning in a home health setting.
- Ability to travel independently throughout Plymouth County, Iowa.
Preferred Qualifications
- Previous experience utilizing the Epic electronic health record (EHR) system.
Skills & Competencies
- Clinical Skills: Proficient in OASIS assessment, care planning, medication administration, wound care, IV therapy, patient education, and comprehensive physical assessment.
- Equipment: Competency with standard home health medical equipment.
- Soft Skills: Excellent verbal and written communication, strong organizational and time management abilities, critical thinking, problem-solving, empathy, adaptability, and the ability to work autonomously.
Shift Details
- Scheduled Shifts: Primarily days, Monday through Friday.
- Hours: 32-40 hours per week, typically 8-hour shifts, 5 days per week.
- Call Requirements: Required participation in a departmental call rotation, approximately one in four nights, weekends, and holidays.
- Contract Length: 13-week assignment.
- Start Date: July 28, 2026.
Licensing Requirements
- Active Registered Nurse (RN) license in the State of Iowa.
- Basic Life Support (BLS) certification.
