We are seeking a dedicated and experienced Home Health Registered Nurse to provide comprehensive, in-home patient care within Plymouth County, Iowa. This role is based out of Le Mars, Iowa, and focuses on delivering high-quality nursing services to patients in their residential settings.
The successful candidate will be responsible for managing a caseload of patients, conducting thorough assessments, developing individualized care plans, and ensuring continuity of care. This position requires a compassionate professional with strong clinical skills and the ability to work independently while collaborating with a multidisciplinary team.
Key Responsibilities
Provide skilled nursing care to patients in their homes, adhering to established care plans and physician orders.
Conduct comprehensive OASIS assessments to determine patient needs and eligibility for home health services.
Develop, implement, and evaluate individualized patient care plans, adjusting as necessary to meet changing patient conditions.
Monitor patient health status, administer medications, perform treatments, and educate patients and families on disease management and prevention.
Maintain accurate, timely, and complete electronic health records in compliance with regulatory standards.
Collaborate effectively with physicians, therapists, social workers, and other healthcare professionals to ensure coordinated patient care.
Participate in the departmental call rotation, providing support for nights, weekends, and holidays.
Required Qualifications
Active Registered Nurse (RN) license.
Minimum of two (2) years of recent experience performing OASIS assessments and care planning in a home health setting.
Ability to travel independently throughout Plymouth County, Iowa.
Preferred Qualifications
Previous experience utilizing the Epic electronic health record (EHR) system.
Skills & Competencies
Clinical Skills: Proficient in OASIS assessment, care planning, medication administration, wound care, IV therapy, patient education, and comprehensive physical assessment.
Equipment: Competency with standard home health medical equipment.
Soft Skills: Excellent verbal and written communication, strong organizational and time management abilities, critical thinking, problem-solving, empathy, adaptability, and the ability to work autonomously.
Shift Details
Scheduled Shifts: Primarily days, Monday through Friday.
Hours: 32-40 hours per week, typically 8-hour shifts, 5 days per week.
Call Requirements: Required participation in a departmental call rotation, approximately one in four nights, weekends, and holidays.
Contract Length: 13-week assignment.
Start Date: July 28, 2026.
Licensing Requirements
Active Registered Nurse (RN) license in the State of Iowa.
