Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year Adult Home Health experience or 1 full year Hospice experience License: Active nursing license required Certifications: CPR, BLS Must-Have: - Proficiency in HomeCare HomeBase EMR and OASIS - Reliable transportation, current driver’s license, and agency-required liability insurance - Comfortable traveling up to 75 miles from home clinic Description: The Registered Nurse provides coordinates and directs the provision of home nursing care according to physician orders. Nursing care is based on agency policies and procedures through the competent application of the nursing process. This role is responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses or Licensed Vocational Nurses. Home visits are integral to this position, along with periodic office meetings. The average number of patients seen per day is 5 to 10, equating to approximately 6 to 8 points per visit. Documentation, travel, and other activities hold different point values. Weekly productivity expectations are set at 30 points. The types of cases include Start of Care, Wound Care, and Follow-up Visits. Weekend requirements may vary, and the candidate should discuss this during the interview. On-call responsibilities will vary; the candidate must be flexible. All documentation must be completed within 24 hours of patient appointments. Candidates should have strong clinical assessment skills and be familiar with various procedures and patient care management. The Registered Nurse must be proficient in infection control precautions and palliative symptom management. The candidate will be responsible for driving to patients' homes daily and attending any required meetings. Declining patients may result in a cancellation of the contract. No guaranteed hours are associated with this position; candidates must be prepared for varying patient census.
