Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice experience depending on the specialty License: Must have license in hand Certifications: CPR, BLS Must-Have: Proficiency in HomeCare HomeBase EMR and OASIS Description: Provides and coordinates home nursing care according to physician's orders. Acts in accordance with agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses or Licensed Vocational Nurses. Candidates must have reliable transportation, a current driver's license, and agency-required liability insurance. Expected average number of patients per day is between 5-10, averaging about 6-8 points. Each type of visit and activity (documentation, travel, etc.) has a different point value. Types of cases include Start of Care, Wound Care, and Follow-up Visits. All documentation must be completed within 24 hours of the appointment. Documentation delays may result in cancellations. Must be willing to travel up to 75 miles from the assigned care center. Comfortable travel is essential as drives may exceed 20 minutes between patients. Weekly productivity points must be met, equating to approximately 30 points which corresponds to full-time hours. Candidates must accept every patient on the schedule, regardless of distance from previous appointments. On-call requirements vary per location and profession. Must be proficient in OASIS and have utilized it within the last 6 months or be willing to take a retraining course. Candidates must be aware this is a home health assignment and will be responsible for driving to patients' homes. Local candidates are accepted. Must not currently be a direct employee of any affiliated location if applying as a traveler.
