Job Title: Home Health Registered Nurse Profession: Registered Nurse Specialty: Home Health Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours Experience: 1 Year of Experience Required Adult Home Health Experience Required Epic Experience Required License: Active RN License Required Certifications: BLS Required Must-Have: Proficient in PICC, Port, Tracheostomy, Pleurx, and patient education. Familiarity with Medicare guidelines related to Homecare and ability to complete OASIS documentation. Description: The position requires a Home Health Registered Nurse working from 08:00 to 16:30. Every 5th weekend is required. No float or call is needed. Proficiency in performing skills such as admission assessment, management of peripheral IVs, central lines, ports, chest tubes (Emerson/PleurX), and home infusion is essential. Candidates must demonstrate strong clinical assessment documentation skills. Experience with colostomy/ileostomy management, discharge planning, and routine visits is needed. Ability to care for feeding tubes, urinary catheters, and perform wound care is necessary. The daily nurse-to-patient ratio will be 16-20 patients, with 5-6 visits per day. Interdisciplinary support will include physical therapy and social services. Special procedures in this role include managing pleurx and tracheostomy care. Equipment used will include a laptop and a cell phone. Documentation must comply with relevant healthcare standards.
