Schedule: Day Shift – 5x8 hours (8:00 AM – 4:30 PM)

We are seeking an experienced Home Health Registered Nurse (RN) to provide high-quality patient care across Baltimore County. This role focuses on adult and geriatric patients in a home care setting, with an expected productivity of 6 visits per day (30 points/week).

• Deliver skilled nursing care in patients’ homes
• Perform comprehensive assessments, admissions, and recertifications
• Manage OASIS documentation accurately and timely
• Administer medications, IV therapy, and wound care
• Educate patients and families on treatment plans and disease management
• Coordinate care with interdisciplinary teams
• Handle case management, discharge planning, and significant condition changes

• IV therapy and phlebotomy
• Wound care and VAC management
• Catheter and feeding tube care
• Tracheostomy and ventilator management
• Pain management and palliative care
• Home infusion and case management experience
