Location: Williston, ND

Start Date: 03-23-2026

End Date: 06-22-2026

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM (Day) with rotating on-call evenings and weekends; holiday coverage required

Hours per Week: 40

Roles & Responsibilities
  • Perform initial comprehensive assessments of clients and families to determine home health needs
  • Re-evaluate and update client nursing needs and diagnoses using health assessment data
  • Initiate, implement and revise individualized plans of care and goals based on diagnosis
  • Administer medications and treatments as prescribed and document appropriately
  • Provide clear, compassionate education and instructions to clients and families
  • Identify and implement discharge planning and needed community resources
  • Serve as case manager when assigned, coordinating care across providers and settings
  • Prepare clinical notes, maintain accurate documentation, and communicate with primary physicians at least every sixty days or as needed
  • Ensure equipment and necessary services are arranged and available
  • Instruct, supervise and evaluate home health aide care provided to clients
Knowledge & Skills
  • Strong patient assessment and clinical judgment skills
  • Experience developing and revising plans of care and care coordination
  • Medication administration and treatment management
  • Proficiency with HomeCare HomeBase EMR (or similar electronic charting systems)
  • Excellent clinical documentation and communication with interdisciplinary teams
  • Case management skills and the ability to prioritize a varied caseload
  • Compassionate bedside manner and family-centered teaching skills
  • Reliable time management and the ability to travel within the service area
