Description:
Provides, coordinates, and directs the provision of home nursing care according to physician orders.
Care is based on agency policies and procedures through the competent application of the nursing process.
Responsible to Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses.
Home visits are required.
Periodic office meetings are necessary.
Must have reliable transportation and a current driver's license.
Agency-required liability insurance is needed.
Average number of patients per day is 5-8.
Contractor will be assigned between 5-10 patients per day.
Patients equate to roughly 6-8 points.
Types of cases include Start of Care, Wound Care, and follow-up visits.
Weekend requirements vary by location and profession.
Is on-call requirement varies by location and profession.
Must be willing to travel up to 75 miles from the home clinic.
Mileage is reimbursed at the IRS standard rate.
The commute to and from the first and last patient is not billable.
Local candidates are accepted at the same rate.
Pending licenses are not accepted; must have a license in hand.

There are no guaranteed hours with this contract.
Clinicians must hit weekly productivity points, which equate to 40 hours per week.
If not receiving enough patients, there may be an opportunity to float to another nearby clinic to pick up patients.
