Home Health Registered Nurse (RN)

Job Overview

We are seeking an experienced Home Health Registered Nurse (RN) for an immediate start. This position involves providing skilled nursing care to patients in their homes, ensuring high-quality patient outcomes and adherence to care protocols.

Responsibilities

• Conduct comprehensive patient assessments and create individualized care plans.
• Perform necessary medical procedures and administer medications as prescribed.
• Document patient care and progress using OASIS E and Homecare Homebase/Pointcare EMR.
• Achieve a minimum of 6 points per day, correlating to 3 to 6 patient visits daily.
• Potentially float to cover other zip codes based on staffing requirements.
• Maintain effective communication with patients, families, and interdisciplinary teams.
• Adhere to all safety and quality protocols while delivering patient care.
• Submit mileage for reimbursement from the first to last patient visit of the day.

Schedule

This is a full-time position with a schedule of 5x8 hours, from 08:00 AM to 05:00 PM. Orientation is provided every Monday.

Qualifications

• Active Registered Nurse (RN) license in Maryland.
• Minimum of 2 years of nursing experience, with at least 1 year in a travel assignment.
• Experience with OASIS E and Homecare Homebase/Pointcare EMR is mandatory.
• BLS certification is required.
• Must have a reliable vehicle, with valid insurance and licensing for personal transportation.

Service Areas

The service area includes Montgomery County, Prince George's County, Charles County, and St. Mary's County. Nurses may be requested to cover additional zip codes as needed based on patient care demands.
