Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.

Work Location: Buford, Georgia, 30518

Coverage area: 60 miles radius from the facility address. (Gwinnet County)
IRS Mileage Reimbursement ($0.76/mile)
Contract Length: 13+ Weeks (Possible Extension)
Start Date: ASAP
Schedule: Monday-Friday, Day Shift
Hours: 5 x 8-hour shifts (40 hours/week)
On-Call/Weekend Requirement: As required by agency needs

Position Overview:

We are seeking an experienced Home Health Registered Nurse (RN) to provide skilled nursing services to patients in their homes.

The ideal candidate will have recent home health experience, strong clinical assessment skills, and proficiency with HomeCare HomeBase documentation.

This role requires independent clinical judgment, excellent time management, and the ability to manage a productive caseload while maintaining high-quality patient care.

Key Responsibilities

• Perform comprehensive patient assessments and develop individualized plans of care in collaboration with physicians and the interdisciplinary team.
• Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and Routine Skilled Nursing Visits.
• Assess patient conditions, monitor progress, and identify changes requiring intervention or physician notification.
• Provide skilled nursing treatments, medication management, patient education, and disease management.
• Coordinate care with physicians, therapists, caregivers, and community resources to ensure continuity of care.
• Maintain timely and accurate clinical documentation in HomeCare HomeBase (HCHB).
• Manage patient caseload efficiently while meeting established productivity requirements.
• Travel throughout assigned territories and assist with additional coverage areas within a 60-mile radius of the facility as needed.

Productivity Expectations

RNs are required to achieve a minimum of 30 productivity points.

• Start of Care (SOC): 2.5 points
• Resumption of Care (ROC): 2.0 points
• Recertification: 1.75 points
• Discharge: 1.5 points
• Routine Visit: 0.9 points

Required Qualifications

• Active Registered Nurse (RN) license in GA.
• Recent Home Health experience within 2024-2026.
• Strong clinical assessment, care planning, and case management skills.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently and effectively manage a field-based caseload.

Preferred Qualifications

• HomeCare HomeBase (HCHB) experience strongly preferred.
• Experience with OASIS documentation.
• Skilled in home health admissions, recertifications, and care coordination.
• Excellent communication, organization, and time-management skills.
• Ability to thrive in a fast-paced home health environment.
