Must-Have:
Must be proficient in OASIS and have used it within the last 6 months.
Must have proficiency in HomeCare HomeBase EMR and have used it within the last 6 months.
Must be open to traveling up to 75 miles from the assigned care center.
Must be open to on-call rotations.
No contractor hours are guaranteed.
Description:
Provides and coordinates home nursing care according to physicians' orders.
Follows agency policies and procedures through the competent application of the nursing process.
Responsible for the supervision and evaluation of Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Conducts home visits and attends periodic office meetings.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Average number of patients per day is 5-8.
Contractor will be assigned between 5-10 patients daily, equating to roughly 6-8 points.
If low patient census impacts financial needs, there is an option to float to a nearby clinic to pick up hours.
Types of cases include Start of Care, Wound Care, Follow-up Visits, and more.
First-timers are not accepted; recent specialty experience is required.
Weekend requirements vary based on location and profession; discussion during the interview is encouraged.
Documentation must be completed within 24 hours of the patient appointment.
Delayed or incomplete documentation impacting billing and follow-up patient care may result in a cancellation.
It is expected that the clinician will accept every patient on their schedule regardless of distance.
Average travel mileage is around 200 miles per week.
Weekly productivity expectations are approximately 30 units.
Participation in weekly office meetings is required.
