Job Title: Home Health Registered Nurse Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum of 2 years required License: RN license in California Certifications: BLS Must-Have: - EPIC experience - Every other weekend availability - On-call availability (1 day/month) - Willingness to float to designated areas Description: Performs routine assignments and develops competence through structured work. Accountable for the assessment, coordination, delivery, and evaluation of in-home nursing care, including direct patient care and patient/family education. Provides holistic and individualized care to all patients in the assigned area. Develops, implements, and coordinates an optimal interdisciplinary plan of care that incorporates psychological, sociocultural, spiritual, economic, and lifestyle factors. Fosters and maintains collaborative relationships between patients, their family or support group, physicians, and other healthcare providers through effective communication. Adheres to policies, industry standards, best practices, and applicable laws and regulations to promote a quality patient experience. Engages in continuous growth and development in professional nursing practice. Skills: Administration of blood and blood products Admission assessment Blood glucose and glucometer management Care and management of peripheral IVs, central lines, and ports Case management Discharge planning and safety assessments Home infusion therapy Medication administration and management Nutritional assessment Palliative symptom management Tracheostomy care Wound care and management Ventilator management and maintenance Urinary catheter insertion and management
