Job Summary

We are seeking an experienced Home Health Registered Nurse (RN) to provide compassionate, high-quality nursing care to patients in their homes. The ideal candidate will independently manage patient visits, perform comprehensive assessments, coordinate interdisciplinary care, and educate patients and caregivers to promote optimal health outcomes. This role requires excellent clinical judgment, strong communication skills, and the ability to work autonomously in a community-based setting.

Key Responsibilities
Provide skilled nursing care to patients in their homes according to physician orders and individualized care plans.
Perform comprehensive patient assessments and ongoing evaluations of health status.
Develop, implement, and update individualized care plans based on patient needs.
Administer medications, wound care, IV therapy, and other prescribed treatments.
Educate patients and caregivers on disease management, medications, home safety, and self-care techniques.
Monitor patient progress, identify changes in condition, and communicate findings to the healthcare team.
Coordinate care with physicians, therapists, case managers, and other interdisciplinary team members.
Maintain accurate, timely documentation using the electronic medical record system.
Ensure compliance with regulatory standards, infection prevention practices, and organizational policies.
Participate in on-call coverage as assigned and provide compassionate, patient-centered care.
Required Qualifications
Active Registered Nurse (RN) license in Tennessee or a Compact Multistate RN License.
Minimum 2 years of recent Home Health nursing experience.
Previous travel nursing experience preferred.
Experience using Epic electronic medical records.
Strong clinical assessment, critical thinking, and care coordination skills.
Excellent communication, organization, and time management abilities.
Valid driver's license and ability to travel between patient homes.
Preferred Qualifications
Three or more years of Home Health nursing experience.
Previous travel assignment in a Home Health setting.
Experience managing diverse patient populations in the community.
Schedule
13-week travel contract
40 hours per week
Monday through Friday
Shift Hours: 8:00 AM – 5:00 PM
On-call: Approximately 7–10 on-call shifts per month.
Additional Information
Candidates must reside outside the local travel radius to qualify for this travel assignment.
At least one prior travel assignment is preferred.
Experience with Epic EMR is required.
Annual influenza vaccination is required.
COVID-19 vaccination is not required for this assignment.
