Home Health Registered Nurse (RN) - Travel Contract

Job Summary

We are seeking a dedicated and experienced Home Health Registered Nurse for a 13-week travel contract at York Hospital in York, Maine. This full-time position offers a unique opportunity to provide comprehensive nursing care within the comfort and familiarity of patients' homes across the York community.

The successful candidate will manage a diverse caseload, delivering skilled nursing interventions, conducting thorough assessments, and developing individualized care plans. This role emphasizes patient education, promoting independence, and ensuring continuity of care for individuals recovering from illness, managing chronic conditions, or requiring post-operative support in a home setting.

Key Responsibilities
  • Perform comprehensive patient assessments, including OASIS documentation, to develop and implement individualized care plans.
  • Administer medications, perform wound care, and manage medical equipment as prescribed.
  • Monitor patient progress, identify changes in condition, and communicate effectively with physicians and the healthcare team.
  • Educate patients and their families on disease management, medication adherence, and self-care techniques.
  • Coordinate care with other healthcare professionals and community resources to ensure holistic patient support.
  • Maintain accurate and timely electronic health records and documentation.
Required Qualifications
  • Minimum of one (1) year of recent experience in Home Health Care.
  • Registered Nurse (RN) licensure.
