Travel Nurse RN - Home Health - $2,438 per week in Roseburg, OR

Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 17:00) Experience: Minimum of 1 full year of Adult Home Health experience or 1 full year of Hospice experience, depending on the specialty. License: Must have license in hand. Certifications: CPR, BLS Must-Have: Recent specialty experience; proficient in OASIS and HomeCare HomeBase EMR. Description: Provides coordinated and directed home nursing care according to physicians' orders. Care is based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits are required, with periodic office meetings. Must have reliable transportation and a current driver's license. Agency-required liability insurance is necessary. Average number of patients per day is between 5 to 8. Each type of visit and activity holds a different point value, with an expectation of achieving 30 points per week. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Clinician is expected to be flexible with travel, willing to cover a mileage radius of 50 to 75 miles. This may result in drives longer than 20 minutes between patients. Documentation must be completed within 24 hours of each appointment. Delayed or incomplete documentation impacting billing and follow-up patient care may result in a cancelation. There are no guaranteed hours for this contract, and clinicians must meet weekly productivity points equating to 40 hours per week. On-call rotation may be required with potential involvement of multiple RNs. All clinicians must ensure they can accept every patient on their schedule, regardless of distance from previous appointments. Local candidates are accepted at the same rate. Approval is needed for any overtime.
