Call Requirement

• Call rotation every 6–7 weeks.
• One week of night call from 5:00 PM – 8:00 AM, including that weekend.
• No night home visits are made from this office.

Patient Load

• Approximately 5–6 patients per day.
• Branch census of approximately 100 patients.

Documentation

• Homecare Homebase (HCHB)

Required Experience

Nursing:

• Home Health experience (Required)
• Hospice experience (Required)

Therapy:

• Home Health experience (Required)

Scheduling

• Monday–Friday, Day Shift

Floating

• Home Health patient visits within the service area.

Dress Code

Nursing:

• Navy Blue or Navy/White scrubs

Therapy:

• Business Casual

Additional Requirements

• Reliable vehicle required for patient visits.
