Ability to assess and educate patients regarding medications and care plans. Description: The position involves providing home health care to patients in their residences. Average visits per day range from 5 to 20. The role includes performing wound care, IV therapy, and patient education. Clinicians will conduct assessments, including post-operative care and chronic illness management. On-call duties are required after hours and on weekends. Travel may be required within a territory covering a 40 to 80-mile radius. Mileage will be reimbursed based on company policy.
