Role OverviewTorrance Memorial Medical Center is seeking a highly experienced Home Health RN to manage a diverse patient caseload in LA County. Unlike traditional "per visit" models, this is an hourly pay model requiring strict adherence to an 8-hour workday starting at 08:00. Candidates must have strong assessment skills, particularly in Heart Failure and wound care.
Key Responsibilities- Caseload: Typically 5–6 patients per day.
- Territory: LA County coverage with a 5–20 mile radius.
- Clinical Duties: PICC line dressing changes (required), extensive lower leg wound care, IV therapy, and anticoagulant management.
- OASIS: Proficiency in OASIS assessment/documentation (recent experience required).
- Work Model: Hourly model; visits must be completed within the scheduled 8-hour shift.
Schedule & Call Details- Weekend Requirement: Every 4th weekend.
- On-Call: Required as scheduled.
- Holidays: 1.5x holiday rate.
- Orientation: 40 hours total.
Additional Unit Information- Staffing Model: Hourly (Travelers are required to work the full 8-hour shift).
- Productivity Expectation: Expect 3 "Starts of Care" in a 12-hour window or 6–7 wound care patients in a standard shift.
- Physical Requirements: Ability to stand for long periods, squat, and climb stairs without assistance.
