Pride Health is actively hiring a Registered Nurse (RN) to join our client's Home Health Team.

 

Location: Columbus, MS 39705

Patient Care Setting: Home Health
Schedule: Monday-Friday, 8:00 AM - 4:30 PM
Productivity Expectation: 30 Points
On-Call Requirement: Not required, but available if desired
Weekend Requirement: Not required, but available if desired
Coverage Areas: Lowndes, Clay, and Noxubee Counties, Mississippi
Mileage Reimbursement: Yes, reimbursed between patient visits
Dress Code: Royal Blue Scrubs

Position Overview:

  • We are seeking an experienced and compassionate Home Health Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes throughout Lowndes, Clay, and Noxubee Counties.
  • The RN will be responsible for conducting comprehensive patient assessments, managing plans of care, performing skilled nursing interventions, and coordinating care with interdisciplinary team members to promote positive patient outcomes and independence.

Key Responsibilities:

  • Perform comprehensive nursing assessments and OASIS evaluations for home health patients.
  • Complete Start of Care (SOC), Resumption of Care (ROC), Recertification, Discharge, and routine skilled nursing visits.
  • Develop, implement, and update individualized plans of care in collaboration with physicians and interdisciplinary healthcare team members.
  • Provide skilled nursing services including IV therapy, medication administration, wound care, wound assessment, and patient monitoring.
  • Perform patient and caregiver education regarding disease management, medications, treatments, home safety, and self-care techniques.
  • Monitor patient progress, identify changes in condition, and communicate findings to physicians and care team members.
  • Coordinate services with therapists, physicians, caregivers, and community resources to ensure continuity of care.
  • Maintain accurate and timely clinical documentation in accordance with agency, state, and federal regulations.
  • Meet established productivity standards while delivering exceptional patient-centered care.

Required Qualifications:

  • Active Registered Nurse (RN) License.
  • Previous nursing experience in a clinical setting.
  • Strong assessment, care planning, and patient education skills.
  • Experience providing wound care and IV therapy.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Ability to work independently and manage a field-based caseload.
