Job Summary Home Health Registered Nurse (RN) located in the West Salem/Keizer area and surrounding regions. This position involves providing skilled nursing care to adult and geriatric patients within a home health setting. The role requires dedication to patient-centered care, adherence to clinical protocols, and the ability to operate independently in patients' homes.
Job Details
Schedule: 8-hour days, 40 hours per week, starting on 08/24/2026.
Required Qualifications: 1 year of skilled home health experience, Oasis experience, valid and active license, certifications in Basic Life Support (BLS), and experience with adult home health patient care, assessment, discharge planning, recertification, wound care, medication management, starting and maintaining IVs, phlebotomy skills, infection control precautions, medication administration (intramuscular, intravenous infusions, subcutaneous).
Preferred Qualifications: Epic electronic health record experience.
Float: No float rotation required.
Additional Information: The traveler is expected to have reliable transportation. Occasional visits within a maximum radius of 50 miles are possible. The facility requires the primary series of COVID-19 vaccination, including one booster. The work involves daily visits to adult and geriatric patients with a typical ratio of up to six patients per day. The role may involve visiting multiple patients efficiently, with the system grouping patient visits to minimize drive time.
