Key Responsibilities of the Registered Nurse - Home Health- Observe, assess, plan, implement, and evaluate nursing support for well, chronic, or acutely ill individuals.
- Work closely with other health professionals/providers to address health needs.
- Coordinate communications and actions of various disciplines related to health and safety.
- Manage medications as prescribed by the physician and ensure related policies are in place and up to date.
- Report progress to the physician when the client's condition changes.
- Complete routine documentation and ensure the maintenance of health status records.
- Direct and supervise licensed practical/vocational nurses and provide staff training.
- Maintain communication with inpatient nursing and discharge coordinators.
Requirements of the Registered Nurse - Home Health- RN License
- Oasis and HCHB experience required
- Experience in home care
