Home Health/Hospice Registered Nurse (RN)

Job Overview

This position involves providing care for patients in their homes as part of a Home Health and Hospice team. The focus will be on delivering comprehensive nursing care, including management of both chronic and acute conditions.

Shift Details

  • Shift Type: Days
  • Schedule: Monday to Friday 0800-1630
  • Rotating weekends and holidays required
  • On-call responsibilities: One weekend on-call per month and one evening on-call per week

Key Responsibilities

  • Provide nursing care to patients with diagnoses including CHF, Diabetes, Renal Disease, Wound Care, and IV therapy.
  • Perform comprehensive assessments and develop care plans based on patient's needs.
  • Conduct OASIS documentation for admissions, recertifications, and resumptions of care.
  • Ensure excellent infection control practices in the home environment.
  • Collaborate with other healthcare professionals and families to ensure patient needs are met.

Qualifications

  • Minimum of 1 year of recent Home Health experience within the last 2 years.
  • Proficiency in OASIS documentation.
  • Wound care experience is required.
  • Current Basic Life Support (BLS) certification (AHA).

Additional Information

  • A typical day involves seeing up to 6 patients, including assessments and ongoing care.
  • Nurses will work collaboratively with CNAs employed in the home health setting.
  • Scheduling is managed by the office; nurses are expected to typically work five days per week unless scheduled for the weekend.
  • Participation in weekend and holiday rotations is expected.
  • All nurses will work with both home health and hospice patients, with relevant training provided for care protocols.
  • Professional attire: Any solid color, clean and professional clothing is acceptable.
