Opportunity:

RN - Home Health Nurse

Job Details:

• 5-week contract with an ASAP start date
• 8-hour day shifts, approximately 8:00 AM to 4:30 PM with flexibility depending on census
• Guaranteed 40 hours per week
• Every 5th weekend and holiday rotation required
• On-call required 2-3 times per month
• Primary location is Wausau Home Health with potential to float to Medford, Antigo, and Wisconsin Rapids on a limited basis
• Manage 20-25 patients with an average of 4.5 visits per day
• Travel ranges from 25-60 miles per day depending on territory, patient census, and staff availability
• Mileage reimbursed at the government approved rate
• Responsibilities include OASIS admissions, all time points, episode management, physician calls, coordination of care, timely EMR documentation, and clinical competencies in wound care, IV, ports, lab draws, and chronic disease management
• Charting experience with OASIS required; experience with Epic Remote Client/Remote Care preferred
• Minimum 2+ years of home health experience required
• Prior travel experience required
• Required certifications: AHA or ARC BLS, active WI or Compact license, valid driver's license; OASIS certification is a plus
• Associate Degree or Certificate required; Bachelor's Degree preferred
• Scrub color: Navy blue
