Description:
The Community Health Nurse (CHN) provides nursing care for the sick and injured in home settings.
The CHN intervenes to promote, maintain, or restore health, prevent illness, and facilitate rehabilitation.
The CHN collaborates with healthcare providers and community representatives to coordinate care at home.
The CHN evaluates individuals’ and families’ responses to interventions.
The role requires the ability to function independently under the general guidance of the Director.
The CHN follows specific guidelines, protocols, and policies.
Duties require independent judgment and action.
The CHN utilizes time management, communication, planning, delegating, and problem-solving skills in providing care.
Critical thinking skills are essential for this position.
Emotional stability in emergency settings is important.
The CHN must demonstrate interpersonal skills and the ability to supervise others.
Confidential information must be maintained at all times.
The CHN develops and maintains records while effectively communicating clinical data verbally and in writing.
Flexibility regarding work hours is required.
Driving an automobile in inclement weather is necessary.
On-call responsibilities are part of the role.
Adherence to customer service and performance standards is expected.
The CHN follows the Patients’ Rights Policy.
Dependability in meeting the demands of the work schedule is crucial.
Requested time off must be noted during the application process.
Requests for time off after submission will not be approved.
No more than five days of requested time off will be accepted.
The candidate must have experience with self-scheduling of caseload preferred.
A history of flexibility in assuming varied assignments is desired.
A Bachelor of Science in Nursing (BSN) is highly preferred.
