Job Summary
Join a dedicated team as a Home Health Registered Nurse (RN) Case Manager, providing essential nursing, rehabilitation, hospice, and personal care services to patients in their homes. This role offers the opportunity to build meaningful relationships with patients and their families, becoming an integral part of their care team while enjoying a high degree of independence.
Based out of the White River Junction, Vermont area, this position serves a broad community across more than 70 towns in Vermont and New Hampshire. You will manage a diverse patient caseload, focusing on comprehensive assessment, care plan development, and coordination within an interdisciplinary team.
The ideal candidate is passionate about delivering compassionate, effective home-based care, valuing both autonomy and collaborative teamwork to enrich the lives of those served.
Key Responsibilities
- Develop, implement, and periodically review patient care plans in collaboration with the patient's physician, the patient, and their family or primary caregivers upon admission and as appropriate.
- Coordinate the patient's care plan through effective interdisciplinary communication and participation in case conferences.
- Provide timely notification to other disciplines regarding patient discharges.
- Conduct ongoing assessments of the patient's clinical, functional, and emotional status to identify problems, monitor progress toward desired goals, and communicate significant changes to the patient's physician and interdisciplinary team.
- Accurately document all evaluations, treatment goals, and objectives in accordance with established protocols.
- Educate patients and caregivers on self-care techniques, medication management, dietary instructions, and other physician-ordered treatments.
- Utilize opportunities for health counseling to promote patient well-being and independence.
Required Qualifications
- Graduate of an accredited School of Nursing.
- Minimum of two (2) years of recent experience in Home Health.
